The EU MDR mandates the use of a Unique Device Identification (UDI) system which will allow the identification and facilitate the traceability of devices. 
The text which is human readable i.e. alpha-numeric identified in the illustration is the traceability or identification used by humans and is known as human readable form. It is mandatory to have it. 
The bar code in the illustration is provided for machines, this is the machine readable information. It is mandatory to have it in UDI.
